Is It Alright for Pain Doctors to Terminate Patients for Using Medical Marijuana?

By David Lawrence, MD

When an individual sees a pain physician, that individual may receive narcotics. Especially if that individual is dealing with chronic pain and no operative solution exists, narcotics may be part of the long term plan.

There are significant potential side effects with opiate medications. This may include constipation, depression, sedation, euphoria, dizziness, fatigue, anxiousness, clammy skin, confusion, respiratory depression, and a slew of others.

One of the biggest issues seen is tolerance and/or addiction with opiates. Tolerance is when the patient's chronic pain condition doesn't change, but the same amount of pain medication doesn't quite provide adequate pain relief any longer.

One of the newer options in 16 states plus the District of Columbia is medicinal marijuana. Treatment with marijuana may offer substantial relief that may decrease the need for high doses of opiates or in some cases provide relief where opiates do not work well.

For example, opiate meds are not a great choice for peripheral neuropathy treatment. The pain is not modulated really well, whereas, marijuana works better for neuropathy.

Medical marijuana does not obviate the need or utility for interventional pain management. When dealing with a herniated disc or a focal medical problem where a pain injection would help, medical marijuana is not the end all answer.

When people are on chronic pain medications with a pain management physician, usually a pain agreement is signed. This "contract" typically states that while a person is under his or her care, that individual will stay away from illicit drugs.

Marijuana remains federally illegal, even medicinally, despite it being legal in 16 states. A significant amount of pain doctors perform urine drug screening on their patients. So if an individual is under contract, undergoes a test, and then tests positive for THC (marijuana's active component), is it proper for the pain doctor to fire the patient?

It's a simple answer as to whether or not the pain doctor has the right to terminate the patient, but not a simple answer as to whether it's appropriate. If the pain agreement states that the doctor has the right to terminate a patient if the drug test turns up positive for narcotics not being prescribed, then that is difficult to refute. If the patient is given the opportunity to rectify their termination by ceasing the marijuana use and re-testing in a few weeks, once again that is the doctor's prerogative.

Ethically, the situation is not so simple. Patients deserve effective pain management, and there is a big push in American not to undertreat. Medical marijuana has shown effectiveness in a number of chronic pain conditions and numerous other conditions such as severe nausea/vomiting and cancer.

Having marijuana continue to be federally illegal and placed into the illicit category puts pain doctors in a very difficult situation. If they test patients for THC and then don't terminate patients who test positive, is it showing bias with regard to other illicit substances?

Some pain physicians don't look at marijuana as illicit because of its medicinal value, so they do not screen for it. If an individual discloses his or her marijuana use to the doctor, the issue becomes the same.

The point here is there really is no clear cut right and wrong answer for the pain doctor to follow. Guidelines need to be individualized. Hopefully once federal views on marijuana change then these ethical issues will become moot.

The biller must be proficient in the [process of generating invoices so that the unpaid bills can be kept to the minimum. They may need to talk to the person in-charge of the agency in order to make sure that the payment does not gets delayed due to some misunderstanding.

Medical coding is completely a different field than billing. The coders have to work with the patient’s data. They are responsible for providing the correct code for the procedure or medication. The coders are concerned with the coding of the tasks associate with the patient so that the relevant bills can be made. In short, they connect the codes with the data base of the patient so that proper billing can take place. Fundamental Facts About The Symptoms Of Scabies

By Neville Sanderson Xavier

Identifying scabies symptoms is not a skill that most of us are born with, unless of course we have experienced these little insects before. This condition in humans represents an infestation by the mite that is called, Scabies. When a person becomes infected with this insect, the infestation occurs on the upper layer of the skin as the microscopic pest burrows deep.

The mites that do survive are able to lay eggs in your skin which will later hatch out causing a larger infestation. Infestations are definitely probable with many people that have scabies, although the percentage of those with a full outbreak is dependent upon many things. People that contract scabies must have physical contact with someone else that has it for an extended period of time.

In other words, that person is contagious for infesting others. The initial several weeks finds you body's immune system developing a sensitivity to the infestation. What happens next is an allergic reaction to the scabies in your skin which is experienced as severe itching. So that means the initial period, of a few weeks, for a first-time infestation means the person can pass it to someone else without their knowing it.

Did you know that you can get scabies in not just one, but several locations on your body? It is possible for scabies to go from place to place because they do not need a body for short periods. If there are certain objects in your house that are shared, scabies can live there and go to different people that touch these objects. Looking like tiny sores, scabies can show up on your scalp or on various other parts of the body. Anatomical locations other than the scalp include your feet, hands, and various private areas. Unless you get treatment, the scabies will form vesicles on your skin which will itch; this will usually prompt you to go to the doctor to get treatment.

Different people are at different levels of risk for getting scabies; this has to do with a variety of factors. Nursing homes, prisons, and a variety of other public locations are breeding grounds for the transference of scabies from person-to-person. Scabies travels only through physical contact, which is definitely possible that places were overcrowding is quite common. In fact, children in developing countries are at a higher risk of contracting scabies due to their living environment. Families in these regions typically see several members of the family with mite infestations. Personal items like linen and towels, especially when used by multiple users, can easily spread to each person. Even though scabies is transmitted through personal contact, sexual activity is not a precursor to its ability to spread from host to host. Having the ability to last apart from the human host for as much as 36 hours, scabies is easily transmittable without human contact. Family members in particular are capable of spreading scabies in a household simply by using similar items each and everyday. Be sure to take anyone in your family that you suspect of having scabies to a physician to not only get a diagnosis, but to start treatment if the scabies has infested their skin.

A lot of people would concur that we now have a lot of health issues that people must handle. And with a maturing society, registered nurses will be in constant demand. If you'd like a fulfilling job that pays huge rewards, think about being a registered nurse. Talk with an occupation consultant for more information.

The Negative Effects Of Hammering Away At That Keyboard

By Yolanda Reyes

Many people frequently say that they spend a great deal of time pounding on their computer keyboards. When they are working on their computers, they would indicate that they hammer away. In fact, there are negative consequences that come for pounding on a keyboard and striking the keys too hard not only for the computer itself but such action can also lead to serious health consequences for the computer user.

Perhaps the most common disability or condition that can arise from striking the keys on a computer too stridently is carpal tunnel syndrome. This debilitating condition is painful and more often than not requires a person to have invasive surgery on the hands, wrists and lower part of the arms.

Another health condition that can arise when a computer user is too hard on the keys is a deadening of the nerves at a person's fingertips. Deadening the nerves at the ends of a person's fingers can be very serious although it may not seem like that significant of a problem on the surface. Because a typical person can feel what is in front, near, or around him on a day to day basis, then he can avoid a good deal of harm to this person. For instance, by instantly detecting the heat from a stove by lightly passing your fingers over and above a particular burner, you can avoid serious damage to yourself if you are working in the kitchen.

In addition to the other ailments and conditions set forth in this article, using too much pressure when striking a keyboard, hitting the keys too hard, can result in a deterioration of joints in a person's fingers. In time a person may even end up afflicted with arthritis as a result of the heavy pounding on a computer keyboard.

When all is said and done, using a great deal of force or pressure on the keys is simply not necessary for a person to do when they are using a computer. The thing is, even with only a light touch on the keys, then the keyboard will function properly. In other words, banging away on the keyboard is not the answer just so you can be effective in your typing and utilization of a computer unit. If you settle by lightly touching your keyboard, then a host of healthy problems both now and into the future are what you can avoid.
